LRA -- Match Feedback/Incident Report

If you've observed a problem or incident, wish to provide feedback, or comment on any particular match, either in Livermore or away, please feel free to submit a Match Feedback/Problem Report (see below) where you can express your observations and opinions.

We may not be able to respond to every report. However, if there is a pattern that develops pertainingto a particular individual referee, we may take further action including requesting that a qualified, USSF Assessor be assigned to observe, counsel and advise the referee, or assistant referee. We are also interested in positive comments as we will be selecting from our local referee community a Referee-of-the-Year and a Youth-Referee-of-the-Year.

Please be as complete, factual, and specific as you can without emotional statements. Our long term goal is to improve as much as possible the recruitment, development, quality, and growth of USSF-registered soccer referees in Livermore so keep those thoughts in mind as you fill out the form.
